RMA Request Form

Returning your In-Warranty product for Warranty repair or Warranty replacement If you are experiencing a problem with your Lumantek product, please fill out and submit the Return Material Authorization (RMA) Request Form. For end users, please enter your RMA Code. Distributors or dealers should login using their ID and Password.

For products ineligible for warranty repairs and product placement, we are not responsible for return shipping fees.
